Management Pearls

Treatment often depends on the specific etiology and context.

For an appropriate diuresis, can often permit return to homeostasis. Be mindful of the rate of normalization in certain cases, such as if there is severe hyponatremia from polydipsia at risk of overcorrection. If has an appropriate sodium diuresis and is not hypovolemic, avoid matching urinary volume losses by replacing with the same volume of isotonic IVFs, as this will cause the polyuria to persist.

Anticipate changes in serum sodium that may need to be corrected by calculating the EFWC of the urine. Recall that free water can be lost in some cases of solute diuresis, such as urea, as this is not an effective osmole.
